Target Audience: Wisconsin social workers, registered nurses and other health care professionals seeking certification to administer functional screens for an agency approved by DHS. Course Objectives: This course is structured in modules that are consistent with the Department’s document titled MH/AODA FS Instructions, which may be viewed online at www.dhs.wisconsin.gov/LTCare/FunctionalScreen/mhaoda/instructions.htm.
Approximate Course Completion Time: The average individual spends approximately 10 hours to complete this course. This course may be completed at the pace dictated by each user; logging in and out of the course over a period of hours or days is permitted. |

Course Requirements: Modules must be completed in order.The online certification course consists of eight certification exams. All exams must be successfully completed (a score of 80% or greater) in order to become a certified CLTS FS screener. |
