Course Description
This course helps you develop a comprehensive understanding of how to administer and complete the Mental Health/AODA Functional Screen (MH/AODA FS) for individuals applying for Wisconsin's mental health programs.

Only qualified and experienced professionals who have passed this online certification course and who meet additional requirements prescribed by the Department of Health Services (DHS) are permitted to complete the MH/AODA FS.

Target Audience: Wisconsin social workers, registered nurses and other health care professionals seeking certification to administer functional screens for an agency approved by DHS.

Course Objectives: This course is structured in modules that are consistent with the Department’s document titled MH/AODA FS Instructions, which may be viewed online at www.dhs.wisconsin.gov/LTCare/FunctionalScreen/mhaoda/instructions.htm.

  1. Overview and History of the MH/AODA FS
  2. The Screening Process and Quality Assurance
  3. Basic Information and Demographics
  4. Community Living Skills
  5. Crisis and Situational Factors
  6. Risk Factors
  7. Diagnoses
  8. Completing the MH/AODA Functional Screen

Approximate Course Completion Time: The average individual spends approximately 10 hours to complete this course. This course may be completed at the pace dictated by each user; logging in and out of the course over a period of hours or days is permitted.

 

Course Requirements: Modules must be completed in order.The online certification course consists of eight certification exams. All exams must be successfully completed (a score of 80% or greater) in order to become a certified CLTS FS screener.

 

Course Completion and Certification: Upon completion, you will receive:

  • A certificate of completion noting 10 hours of continuing education. The certifcate of completion will be emailed to you approximately 30 minutes after completing the course.
  • Information on how to request access to the Functional Screen Information Access (FSIA) system.

How to Access This Course: After you receive your logon ID:

  1. Select the “My Curriculum” link in the left navigation bar.
  2. Enter your ID and click OK.
  3. Select the “Mental Health/AODA Functional Screen Certification Course.”
  4. Click the module code to the left of the module name.
  5. Select the “Module Content” tab to begin the Introduction to the course.
