Individuals successfully completing one or more of the following courses can be found on the Dementia Registry:

• Dementia Generalist for Crisis Responders
• Dementia Generalist for Healthcare Providers
• Dementia Specialist Designation
• Challenging Behavioral Specialist Trainer
• Challenging Behavioral Specialist

If you are looking for a complete list of your own dementia training (with CCDET), click on My Transcripts. You will need to log in with your personal ID.

Individuals are listed in alphabetical order. Completed courses are shown, along with the course completion dates, for each individual.

NOTE: For privacy reasons, the Dementia Registry does not publish personal identifying information. When two or more individuals with the same name are listed on the registry, contact UW Oshkosh CCDET at dementiacare@uwosh.edu for further verification.The registry contains the names of individuals who have successfully completed dementia related training developed and delivered by UW Oshkosh CCDET on or after February 2, 2015. This registry does not replace industry standards and regulations for hiring. Processes in hiring must continue to be followed.
